The report form
One page, six sections. PDF to print, Word to fill in on a computer.
Download the form below: PDF to print, or Word to fill in on a computer. It has six short sections, and you already have most of the answers from your weekly checks.
Class at a glance: how many students are on track, behind, stuck or not started. Take the numbers from your class dashboard.
Students who needed help: one line each, with the problem, what you did and the result so far. For example: “Mina: never logged in. Gave the login in person. Logged in on Monday.”
Use first names or initials only. Never write a full name, a username or a password.
Technical problems: what broke, who you told, and whether it is fixed.
What students found hard: topics, English terms, and what students told you.
Parents contacted: who and why, only if you did. Write to a parent when talking to the student has not helped. Lesson 7 shows how to send a parent a copy.
What you need: questions, or help you are asking for.
Send the report by email, or give it to your school's representative.
You're done when you can fill in the form in about ten minutes, and you know where to send it.
